Peru Visa Checklist

Tourist Visa Application Details:

1.   Original passport with at least 1 year validity from the date of intended travel along with the original previous passport (if any) 

2.   2 set of visa application form completely filled in block letters and with black ink

3.   3 latest passport size white background photographs with face close up (do not staple the photograph)   

4.   Covering letter from the applicant stating name, passport number and the purpose of travel

5.   Latest original personal bank statement for the last 6 months (i.e. bank's stamp on every page)

6.   Personal Income tax return of last three years

7.   Relationship proof with the company (i.e. MOA if Director, Partnership deed if Partner, Proprietorship certificate if Proprietor and salary slips of last 3 months in employed)

8.   Hotel reservation voucher from Peru

9.   Return air ticket and day to day itinerary
